We seem to be having problems with the download files. You can probably just print this page by choosing File-Print from your browser window. Or, you can select the text with your mouse and copy it into a word-processing program. Use a fixed-type font like courier, and the alignments should remaind the same. Print out the form and fill it out with the appropriate information. If you're coming to the conference, you can give us the form and the check at the registration table.
